Shelton man injured in Highway 101 car accident

A 39-year-old Shelton man was taken to Mason General Hospital with shoulder injuries after he lost control of his Chevrolet K1500 on U.S. Highway 101 south of Brinnon early Friday morning.

The car accident occurred around 3:21 am when the man's car went off the road, over a guardrail an into a ravine. The car rolled several time before landing on its rear end with it's front end facing upwards.

The Highway Patrol believes that driver inattention may have been the cause of the auto accident.

Family awarded $114 million in nursing home neglect case

The family of Juanita Jackson, 76, was awarded a $114 million in a case of nursing home abuse and neglect that caused her death.

Jackson was moved to a Florida nursing home in March of 2003 for rehabilitation. According to the lawsuit, the nursing home knew that Jackson was a fall risk but did not take preventative measures to protect her. Jackson suffered a serious fall after being at the nursing home only two weeks and suffered a traumatic brain injury and fractures to her upper arm.

Jackson never recovered from the fall and she developed bedsores and suffered malnutrition and dehydration before she died.

Des Moines man convicted for preying on vulnerable elderly

Thanh Diep, who preyed on vulnerable and elderly women, faces 14 years in prison when he is sentenced on Friday morning.

According to a report in The Seattle Times, Diep pleaded guilty to stealing cash and jewelry from seven different women. He also stole Wonedith Peters', 86, pet Chihuahua dog which he gave to his girlfriend.

Diep had a history of similar crimes and spent two years in prison in 2000 for residential burglary, second-degree theft and obtaining a signature by deceit.

Greyhound bus accident kills 6, injures 34

Six people were killed and 4 people were critically injured in a greyhound bus accident in California reports the Fresno Bee.

The bus accident happened around 2:15 on Highway 99 near Fresno.

According to the report, the bus, which was traveling from Los Angeles to Sacramento, apparently hit an overturned SUV which was in the innermost lane of the highway and then may have collided with another vehicle before losing control, going off the road and running into a large tree.

One report said that one of the dead may have been in the SUV.

The National Highway Transportation Safety Board is investigating the cause of the motor vehicle accident.

Idaho man killed when tractor rolls

KTVB.com reports that Herbert Weatherford, 71, of Dreary, was killed in a tractor accident

According to the report, the man was driving his tractor up a steep hill on when it slid and rolled at least four times. The report said that Weatherford was an experienced logger and heavy equipment operator.

The man was taken to St. Joseph Regional Medical Center in Lewiston where he died a short time later.

Baja Motorsports recalls Mini Bikes and Go-Carts due to burn and fire hazard

Baja Motorsports along with the U.S. Consumer Product Safety (CPSC) commission has issued a voluntary recall of approximately 308,000 go-carts and mini bikes because they pose a burn and fire hazard.

According to the recall notice, there was one instance of a serious burn injury due to a leaking or detaching gas cap. The notice also said the throttle can stick and pose an acceleration hazard to users. There have been 25 reports of injuries due to the face or other body parts because of a sticking throttle.

Man killed in pit bull dog attack, 3 others injured

A 71-year-old man, William Parker, died after two pit bull dogs attacked him while he was walking. The man suffered severe dog bites and then went into cardiac arrest and died after the attack.

Parker's daughter was hospitalized in critical condition after the dogs attacked her.

Another man, Kevin Stringell, also suffered dog bites.

The dog attack occurred at 230 North Manassas Street in downtown Memphis, TN.

Two firefighters who came to the man's rescue were also attacked by the dogs and they were treated at a local hospital and released.

The owner of the dogs has been arrested for violating the sex offender registry act. The dogs have been impounded.

Every year, approximately 885,000 require treatment after a dog attack and approximately 16 people are killed. Explosion at Sedro-Woolley shop sends 2 men to Harborview Medical Center with burn injuries

Two men were critically injured in an explosion at a Sedro-Wooley motor vehicle repair and service shop. The accident occurred late Sunday night. The source of the explosion is reported to be a bottle of methanol fuel. Methanol is commonly used in combination with gasoline or nitrous oxide to fuel cars

According to a report by KOMO News, the accident occurred at Funk Racing at 631 Sunset Park Drive. Funk Racing builds and services race cars.

The two men were airlifted to Harborview Medical Center in Seattle for treatment of burn injuries.

Wrongful death lawsuit filed in Olympia claiming bus driver was negligent in fatal accident

The estate of Robert Smith, 42, of Tacoma, has filed a wrongful death lawsuit against Intercity Transit Authority, the City of Olympia and a bus driver, Betty Bridges, in the motorcycle accident in which Smith and Tammi Small, 44, of Olympia, were killed.

According to a report in The News Tribune of Tacoma, the bus driver was negligent and did not come to a complete stop at the intersection of Central Street and Ethridge Avenue, thus causing the fatal accident. The wrongful death lawsuit also claims that the City of Olympia had failed to trim the trees and bushes at the intersection.

The police report on the investigation of the accident states that Smith was speeding before the accident. Toxicology reports also show that Smith had methamphetamine in his system.

Washington state has a wrongful death statute which allows the family of a person who has died due to the negligence of another to file a lawsuit on the behalf of their deceased loved one. CDC says, "1 in 25 outbreaks of foodborne illness linked to fresh guacamole and salsa"

The Centers for Disease Control (CDC) reports that almost one in 25 outbreaks of foodborne illness such as salmonella, norovirus and E-coli can be linked to fresh guacamole or salsa served by restaurants and delis.

Guacamole and salsa are more easily contaminated than some foods because they contain ingredients that come from many sources and they also contain many raw ingredients. If they are not prepared using sanitary prep conditions and if they are not properly refrigerated, bacteria has a chance to grow. Both dishes are made in large batches, so even a small amount of bacteria can infect many diners.

According to the CDC, in 20008, more than 1,400 people were sickened by salmonella and the source of the outbreak was traced back to Serrano and jalapeƱo peppers used in salsa. The CDC note that outbreaks of foodborne disease due to salsa come from the commercially prepared salsas that one purchases at grocery stores but from homemade salsas prepared at restaurants and delis.

Foodborne illness can cause vomiting, acute stomach cramping and diarrhea. In some cases, victims require hospitalization. In extreme cases, some victims suffer severe complications such as kidney disease.

A 24-year-old Ridgefield man was seriously injured in a pile driving accident at the Toutle sediment dam project on Friday afternoon.

According to a report on The Daily News Online, the man was airlifted to Southwest Washington Medical Center in Vancouver after the construction accident.

Pile-driving involves driving piles into soil when building a foundation for buildings or structures. The report said the accident involved a log that construction workers were setting into place.

Woman injured in dog attack at apartment in Seattle's Beacon Hill

A 65-year-old woman was injured when she was attacked and bitten by a pit bull dog on Saturday morning in Beacon Hill reports The Seattle Times.

According to the report, the woman went into an apartment unit owned by her daughter when she was attacked and bitten by the dog.

The Seattle Police responded to a 911 call and took the woman to Harborview Medical Center where she was treated for her injuries.

Windsurfer injured in Lake Coeur d'Alene boating accident

A windsurfer suffered injuries in a boating accident on Lake Coeur d'Alene around 4PM on Friday afternoon.

The man suffered a deep laceration on his calf when he was sucked under a party boat, The Osprey, and was sucked under the boat and got cut by either the boat's propeller or engine.

Man sentenced to 2nd-degree manslaughter in mother's death caused by bed sores

Christopher Wise was convicted of second-degree manslaughter in the death of his 88-year-old mother who was found by King County sheriff's deputies covered with bed sores and lying in a filthy bed.

An autopsy performed on Ruby Wise, Christopher's 88-year-old-mother, determined that she died due to complications from bed sores.

Wise's defense was that his mother wanted to die at home and that he was doing the best that he could to care for her. Prosecutor's told the jury that Christopher Wise lived off his mother's income and and failed to care for her properly.

The jury failed to convict Wise of second-degree murder or fist-degree manslaughter. He was sentenced to three years and three months in prison.

Backhoe driver seriously injured in bridge accident

A backhoe operator was seriously injured in a construction accident on a rural Mason county bridge 12 miles west of Shelton around 10am on Thursday morning.

According to a report by Seattle's KOMO News, the man was injured when the backhoe rolled off a bridge on West Shelton Matlock road.

The backhoe driver was airlifted to Harborview Medical Center in Seattle with serious leg injuries.

Investigative report finds that criminal charges for nursing home abuse may take years

Even after a nursing home receives a citation for the abuse or death of a patient, it some times takes a very long time for criminal charges to be filed, if ever.

In Kentucky, in May of 2009, James "Ronnie Duncan" fell in the nursing home where he lived but he was not treated for three hours after the fall and he died because of traumatic brain injury and bleeding in the brain. Now, eleven months later, a citation has been made against the Johnson Mather Nursing Home where he lived but no decision regarding criminal charges has been made.

This is not an isolated case. Investigations in wrong doing by nursing homes often languish for years going by before a decision about criminal charges is ever made.

Consumer Product Safety Commission may ban drop-side cribs

The Consumer Product Safety Commission (CPSC) proposes new rules governing the sale, re-sale and use of drop-side baby cribs. This year, there has been numerous recalls of cribs across the U.S. because the drop-side can detach when hardware breaks can and a child can become entrapped, suffocate or fall out of the crib.

Pottery Barn Recall

The most recent crib recalls have been from Pottery Barn after there were 36 reports of cribs malfunctioning or sides detaching which resulted in injuries to six children. The retailer has recalled 82,000 cribs because of strangulation, suffocation or fall risk.

The CPSC proposes rules, which would go into effect next year, calling for banning sales of drop-side cribs from stores and banning their use in hotels and daycare centers.

Safety Precautions for Parents

The CPSC provides the following safety tips for parents who have drop-side cribs:
  • Do not use a crib with missing, loose or broken parts.
  • Make sure crib components are tightened regularly.
  • Make sure the crib sides move up and down smoothly.
  • Make sure the sides do not become disengaged which could lead to a space which might entrap a child.
  • Do not use a crib that is more than 10 years old.
  • If you have a problem with a crib or question about it, use the Consumer Product Incident Report.

Yelm boy critically injured when engine hoist falls on him

A 5-year-od boy from was critically injured when a 200-pound engine hoist fell on his stomach near Yelm.

The accident occurred around 3:45 on Wednesday afternoon in the 16000 block of Kalmia Court Southeast.

The boy was taken by air ambulance to Mary Bridge Children's Hospital for treatment.

Washington Department of Health: more prescription drug related-deaths than car accident fatalities in 35-54 year olds

A Washington Department of Health study finds that more people in the 35-54 year old age group die due to accidental prescription drug overdose than in car accidents.

According to the report, prescription overdoses increased 90% between 2003-2008. The report cited 505 fatalities in 2008 due to prescription drug overdose. It also said that 646 people were hospitalized. Methadone is one of the most lethal drugs when taken more than is prescribed because it stays in the body even after the pain relieving effect is gone.

Bizarre head-on motor vehicle accident kills 1, critically injures 2

Oregon authorities are investigating a motor vehicle accident in which two brothers, driving in opposite directions, crossed the centerline and ran head-on into each other.

According to a report on KTVZ.com, Westun Lee Harvey, 20, is in critical condition after the accident and his wife, Rosario Ester Harvey, 28, was killed at the car accident scene.

The collision occurred around 7:10pm, approximately 14 miles south of Hood River when John Arthur Harvey, 25, of Mt. Hood, driving southbound in a tow truck on Highway 35, collided head on with a northbound Toyota Corolla driven by his brother Westun Lee.

It appears that the crash was deliberate because both vehicles crossed opposite lanes of the highway right before the collision.

Boy critically injured in Washougal River swimming accident

Kea Rodrigues, 14, of Wilsonville, was saved from drowning on the upper Washougal River at Dougan Falls after his leg got trapped between boulders in the river.

Bystanders reached the boy when they saw him struggling in the water and kept his head out of water despite the water reaching to his nose. They kept the teen alive until Skamania County Sheriff's divers could rescue him.

Rodrigues was taken by Life Flight to Southwest Washington Medical center in Vancouver before being transported to Legacy Emanuel Children's hospital. A web-site set up to keep friends up-to-date on Rodriques' condition says he is currently on a ventilator after his heart stopped several times.

Redmond man killed, father injured, in Othello motorcycle accident

Kenneth C. Owens, 55, of Redmond, Washington was killed in a motorcycle accident near Othello on Wednesday afternoon. His father, Robert E. Owens, 76, also of Redmond, was listed in critical condition with hand and internal injuries at Kadlec Medical Center in Richland.

According to a report in the Yakima Herald.com, John Mosley, 61, of Cle Elum, crossed the centerline line on State Route 17 around 2:15pm and hit the motorcycles driven by the Owens'. Another motorcycle rider, Michael Willard, 73, of Freeland, was not injured but his motorcycle was damaged by flying debris.

Police are investigating the cause of the motor vehicle accident but they do not believe alcohol or drugs were involved.

QFC stores recalls broccoli salad because of walnuts

Quality Food Centers (QFC) has issued a recall of their Broccoli Raisin Salad that they sold by the pound in the deli service department on July 10th.

According to the recall notice, the salad may contain walnuts though walnuts are not listed as an ingredient on the product label. Some people are severely allergic to walnuts and ingestion of the nut may cause a serious or life-threatening allergic reaction.

QFC says the salad was sold in stores in Oregon and Washington . QFC has offered to provide a refund or a replacement product.

Fife man injured in Cle Elum motorcycle accident

Michael G. Vicente, 26, of Fife, suffered a shoulder injury when he was involved in a motorcycle accident in Cle Elum.

According to the Yakima Herald.com, Vicente was traveling southbound on Oak Street, the Interstate 90 overpass, when he crossed the centerline and hit the guardrail.

Vicente was taken to Emergent Care in Cle Elum for treatment.

Child taken to Harborview after fall from 2nd story window

A 22-month-old baby is in serious condition at Harborview Medical Center in Seattle after a fall from a window on the second story of a building in Martha Lake. Martha Lake is a community west of Mill Creek and northeast of Lynnwood.

According to the report in The Seattle Times, this child fell through a window that had a screen in place. Often, parents have a false sense of security when screens are in place but screens are not strong enough when a child leans against it.

Annually, Harborview Medical Center treats between 50-60 children in the greater Seattle area for falls from windows. These small patients are nick named "window jumpers."

Falls from windows can result in severe head injuries, brain injuries and often death.

2 people feared dead in PA duck boat accident

The Seattle Times reports that two tourists from Hungary, a 16-year-old girl and a 20-year-old man, are feared dead after a barge ran into an immobilized "duck" boat on the Delaware River in Pennsylvania.

The boating accident occurred around 2:30pm after the duck tour boat was adrift after a small fire occurred on board and the engine was shut down. 35 people were successfully rescued after the accident.

The National Transportation Safety Board is investigating the accident and in the meantime the Ride the Ducks tours have been suspended across the nation including in Seattle where the tour is very popular. The Ducks tours provide a tour both on land and in the water on a authentic WWII amphibious landing craft. The NTSB investigation will determine whether proper safety procedures were followed and whether there were enough life vests per passenger.

3 Coast Guard personnel killed in helicopter crash off of La Push, WA

A Coast Guard helicopter crashed off of La Push on Wednesday morning killing 3 crew members and seriously injuring another reports The Seattle Times.

According to the report, the injured crew member was airlifted to Harborview Medical Center in Seattle. A hospital spokesperson says that the injured man was in stable condition.

It is believed that the MH-60 Jayhawk helicopter may have hit power lines, causing it to crash.

Members of the Quileute Nation helped in the rescue effort.

The helicopter and crew were based in Sitka, Alaska and were returning to Alaska from Elizabeth City, N.C. where the helicopter had undergone maintenance.

10th train track fatality this year in Washington

King5.com of Seattle reports that a man was killed by a freight train in Centralia early this morning. The man was crossing the tracks when he was hit by the train.

This is the 10th fatality in Washington state this year involving people who have been hit by trains. Of the 10 deaths, 9 of the people were considered to be trespassers and one was at a pedestrian crossing.

Pedestrians are often ignorant of the dangers that railroad tracks pose. It is often devastating to train engineers to be involved in a collision in which a pedestrian is killed. Pedestrians often don't realize that trains are traveling at speeds of up to 80 mph and that they can not stop for a pedestrian.

Pedestrian Safety Tips

  • Be aware that trains overhang the tracks by at least 3 feet on either side of the track. Also, loose straps or other equipment may overhang further. STAND BACK.

  • Do not attempt to hop on and ride train equipment at any time.

A California jury awarded plaintiffs in a nursing home neglect lawsuit against Skilled Healthcare Group $671 million in damages over alleged violations of California law at 22 of its nursing homes. The California jury found for the patients who had banded together in a class-action suit of 32,000 patients who were patients at one of 22 of the Skilled Healthcare facilities between 2003 and 2009.

The jury, in finding for the plaintiffs in the lawsuit, found that Skilled Healthcare took shortcuts in patient care for their own profit. One of the main issues in the lawsuit is that California law requires that nursing homes provide 3.2 hours of patient care per patient each day, a standard Skilled Healthcare says they met.

The for-profit motive of some nursing homes means that they take shortcuts on patient care for their own bottom line. Signs of nursing home neglect can include:

SeaTac man critically injured in fireworks accident

A 64-year-old man was critically injured when a homemade fireworks device exploded, causing pieces of concrete to fly in the air and hit him in the head. The unidentified man was taken to Harborview Medical Center in Seattle with life-threatening injuries.

According to a report in The Seattle Times, the fireworks accident occurred in the 2800 block of S. 138th Street in SeaTac when the victim's roommate built a device using a bunch of sparklers and then put the device inside a cinder block to brace it.

Using illegal fireworks can result in large fines or criminal charges. Illegal fireworks include firecrackers, all bottle rockets, missile type rockets with fins and fireworks with sticks or fins. Types of illegal explosives include M80s, M100s, cherry bombs, tennis balls, pipe bombs, homemade fireworks, and large dangerous explosives.

2 people killed,1 injured in Pend Oreille lake boating accident

Two people are were killed and one person was injured in a boating accident on Pend Oreille lake near Sandpoint, Idaho late Saturday night reports KHQ.com.

According to the report, the boating accident occurred around 11:30pm under Long bridge. Witnesses said the boat, which was described as a "speedboat", was speeding before it crashed into the cross members under the bridge.

The report said the people that were killed were a married couple in their 30s. An unidentified man was treated at a local hospital and was expected to be released on July 4th.

Woman wins $7 million in nursing home neglect lawsuit

A Lexington, Kentucky jury awarded Grace Fugate $7 million in finding that Hillcrest Nursing home was negligent in the accident that damaged her knee.

Fugate entered the nursing home in July of 2003 when she was only 67 to recover from a knee surgery. In August of the same year, she asked for help going to the restroom but an nursing aide refused to help her and told her she could help herself. It was at this point that Fugate fell.

After Fugate fell, she lost so much blood that she had to be resuscitated. Her knee never recovered and she eventually had to have her leg amputated.

In her nursing home negligence lawsuit, Fugate's attorney argued that the nursing home aide was unqualified and had a poor work history. The jury agreed with this argument and made the award to Fugate.

Salmonella outbreak linked to Sasquatch music festival

Seven people who came sickened due to salmonella illness all attended the Sasquatch music festival over the Memorial Day weekend reports Grant County health officials.

Health officials have not determined the source of the bacteria at this time. Salmonella is a foodborne illness which can be transferred between humans and also animals. Salmonella infections, however, usually occur when people consume foods in which the bacteria is in high concentration. It is often found in fowl or other meats that have not been correctly prepared and in raw eggs.

Every year, approximately 142,000 Americans become infected with salmonella and approximately 30 die.

Fireworks accident injures 3 firefighters

Three fire fighters were injured, one seriously, when the fireworks display they were helping set up exploded in Mountain Home, Idaho.

The fireworks accident occurred around 1:30pm at Desert Canyon Golf Course. The most seriously injured of the three firefighters was said to be in stable condition at the hospital. The two other firefighters were treated and released.

Mountain Home Mayor Tom Rist said that the explosion was probably touched off by static electricity. The Fourth of July fireworks for the city has been canceled.

2 Ellensburg daycare centers temporarily closed due to E-coli illness

Dr. Mark Larson of the Kittitas County Public Health department says that two Ellensburg daycare centers have been temporarily closed after five children were sickened with E-coli illness.

According a report by Seattle PI.com, the daycare centers were closed after several children who attended the centers became ill. One child who did not attend one of the centers was hospitalized with the illness but has since recovered. The decision to temporarily close the centers was one way to halt the person-to-person transmission of the illness.

E-coli is a food-borne and feces-borne illness that can be transmitted when people do not follow proper sanitation and hand washing procedures. It can also be transmitted through undercooked ground beef, unpasteurized juices and cured meat.

The elderly and the young and people with compromised immune symptoms are especially susceptible to serious side effects of the illness.
